Options
We are constantly working to become credentialed and contracted with all insurance companies. Complete the form below and we will verify your benefits before your screening appointment. Please upload a picture of the front and back of your insurance card. With most insurance we have to receive prior approval for your treatment. The insurance company may deny payment. When this happens, we will work with you to get your treatment approved, you will have to pay for your treatment until your treatment is approved.
In addition to commercial insurances, we are an approved Medicaid, Medicare, VA (TriWest/TriCare) service provider.
